what would it take to be able to say, lower a drawbridge only while turning a wheel? with the mouse actually turning a wheel, not just clicking a hotspot.

Not much connected, but as you mentioned this KeyGuard Plugin -which I haven't used yet: is it able to make something like the control of Fahrenheit Indigo Prophecy? (I hope you know that game)
